DHR contracts with Fieldprint to provide criminal history checks through online applications and live scans of fingerprints. The cost is $47.20 per application. To submit your application, please visit the Fieldprint website.

Applicants, except DHR contractors, seeking to have their fees paid by DHR should contact their local DHR office for pre-approval prior to completing an application.

DHR contractors seeking to have their fees paid by DHR should complete the DHR CHC 2208 Vendor Certification Form. Print, sign and notarize the form. Then, email the form to criminalhistory@dhr.alabama.gov for pre-approval prior to completing an application.
